We have to think out of the box, and do something we have never done before - unite and cooperate. Only through unity and cooperation, and collaboration, can we achieve prosperity and abundance for all. The ONE SMALL TOWN initiative has been building this structure since 2005. We are ready. Very excited to work more with ACEH and AKCH2 on partnerships, data sharing, and care coordination between state systems and Continuums of Care. 👏 From Anchored Home: "ACTION 4: Expand data gathering from public systems such as prisons, child welfare and health care institutions using Coordinated Entry to ensure those at risk of homelessness are identified and prioritized for housing interventions upon exit from public systems. Goal: Formalize relationships with no fewer than 100% of state and local public agencies and 60% of local organizations that oversee prisons, child welfare and health care to integrate Coordinated Entry into discharge planning for individuals at risk of homelessness."
