Acute Care Academy: Carrie Kronberg/Tyler Niesen @St. Joseph's in Bellingham, WA Their transition program is structured around the nurse as: Clinician/Advocate/Leader/Educator/Collaborator/Innovator I liked the structure of their program - they have AC101 and AC102 courses, the content which has been built around a 'gap assessment' done with the residents. AC101 is primarily didactic followed by AC102 which is grounded in simulation based on the AC101 content. Sweet....attention to hands AND head. They meet with their grads weekly to see how things are going - this investment (which is see as a thread throughout ALL the programs involved in the ANCC Accreditation Programs) triangulates the graduates with managers, educators and preceptors - a well rounded TEAM approach to transition. What was most fascinating about this program was the 'bring it to the grads' approach of the transition coordinators - there are only 2 of them, but they rotate through all the hospital units where the residents are transitioning, bringing equipment TO the units to teach them skills in the nurses station. They spend an inordinate amount of time face-to-face with the grads (they call elbow to elbow support) and use ONE 12 hour shift/rotation to remove them from practice to provide theoretical support in the classroom - this allows them to come in relatively budget neutral. They suggest that the success with their program is because they 'get their scrubs on' and work at the bedside with the grads (this is on top of the preceptorial support the grads receive).
