Great news to share! Our team has been delving into the intricacies of intraocular inflammation (IOI) in neovascular age-related macular degeneration (nAMD) patients undergoing treatment with brolucizumab. In our recent study, we meticulously gathered and analyzed aqueous humor samples from 96 patients receiving brolucizumab, with subsequent IOI development observed in 19 individuals. Notably, our investigation unveiled heightened levels of P-selectin, TNF-α, and IL-1α in the aqueous humor of those who later experienced IOI. Additionally, elevated serum MMP-9 and reduced plasma Th2 cell percentages were identified in IOI patients. Our findings underscore the association between specific biomarkers in the eye and bloodstream and the development of brolucizumab-related IOI in nAMD patients. It's important to note that we utilize brolucizumab for active nAMD cases, particularly those complicated with massive hemorrhage or polypoidal choroidal vasculopathy (PCV) with grape-like polypoidal lesions. 2,237 Patients, 11 Hospitals, four HCC Criteria: A Comparison Study A recent study, conducted across 11 South Korean hospitals, compared the diagnostic performance of four hepatocellular carcinoma (HCC) diagnostic guidelines and readers' judgments. The study by Jeong Min Lee and colleagues was retrospective, involving 2,237 patients at risk for HCC with 2,445 focal liver lesions (FLLs). FLLs were reviewed according to four guidelines: AASLD/LI-RADS, KLCA-NCC, EASL, and APASL, assessed on accuracy, sensitivity, specificity, positive predictive value, and negative predictive value. Patients with chemotherapy-induced ovarian function failure (CIOFF) may experience ovarian function recovery (OFR) leading to deteriorating clinical outcomes with aromatase inhibitors. These findings underscore the importance of adequate OFS in patients with CIOFF who receive treatment with an aromatase inhibitor. This is a really cool case. A 66-year-old female with #NSCLC receiving #durvalumab presented with a growing right para-mediastinal mass (7.6 x 7 x 9.5 cm) and a new nodule (10 x 9.5 mm) in the right upper lobe (#RUL). #Aliya Pulsed Electric Field (PEF) energy was delivered to the new nodule via a percutaneous needle at the time of confirmational biopsy. Following the procedure, durvalumab was continued for 6 months. At 3-, 6-, and 12-months post-Aliya PEF, the progression was arrested in both the mediastinal and ablated nodule, converting the patient to stable disease. 📃Scientific paper: A case report: a renal squamous cell carcinoma presenting as a nephrocutaneous fistula Abstract: Background Primary renal squamous cell carcinoma is a scarce entity, and its presentation as a discharging sinus is still exceptionally rare. We hereby report a case of a renal cell carcinoma in a young gentleman. Case presentation We report a case of a 20-year-old gentleman who presented with clinical features suggestive of left side xanthogranulomatous pyelonephritis (XGPN). He had undergone multiple endourological procedures for left side renal calculi in the past. We present the clinical presentation, diagnostic dilemmas and treatment for this patient. We performed an open radical nephrectomy for this patient. Our case report is unique from the point of view of nature of presentation to the final histopathological outcome in terms of age group of the patient. Here we present the clinical management and dilemmas in the management of this unusual and rare case. Conclusions Our case report and presentation has taught us that we as clinicians must have a high index of suspicion for a renal malignancy when treating patients who present with pyonephrosis associated with long-standing calculi diseases, irrespective of the age group of the patient. Honoured to have been invited to be moderator of the workshop on the patient experience of immunomodulation and rejection organised by ESOT - European Society for Organ Transplantation. Around 25% of organ transplant recipients encounter acute rejection within the initial year post-transplantation. Over the long term, the threat shifts to chronic rejection, posing a risk to patient survival even with immunosuppressive drug treatments. The topic is very relevant to pulmonary arterial hypertension patients as lung or heart-lung transplant represent an important treatment option for patients with who are refractory to optimized medical therapy. I am myself a former pulmonary arterial hypertension patient, 21 years post double lung transplant.
