LEST WE FORGET    I am (so far as other constraints permit) making a daily post of process and other safety events which have happened on that day in previous years.  I will intersperse these with mental health and safety incidents, since my twin passions are process safety and mental health and safety.    This is to promote thinking and is based on the theory that, by and large, we don't invent new ways of killing people: we fail to adequately learn from the mistakes of the past.    On this day (21st January) in 1969, the Lucens reactor accident occurred. https://lnkd.in/e3Ndx2pG What can your organisation learn from this?
