A recent study published in JAMA Network Open showed 1 in 20 patients who visited a Tennessee emergency department screened positive for housing insecurity or homelessness: https://lnkd.in/e3a6E8yU These findings highlight the urgent need to assess social determinants of health (SDOH) and connect patients to resources that can help. And here's the kicker: addressing SDOH isn't just good practice—it's a new regulatory requirement for health plans and providers. At Watershed Health, we're committed to empowering healthcare providers to address these needs effectively. Our platform enables seamless SDOH screening together with connections to community resources, ensuring comprehensive, whole-person care. Together, we can create a healthier and more equitable future for all.

🌟 Highlight from the Serious Illness Conversations (SIC) Workshop in Feb 2024! 🌍 In the Feb 2024 run of our 1.5-day SIC workshop, healthcare professionals (HCPs) immersed themselves in SIC, gaining invaluable skills to navigate crucial dialogues with patients and their families. We received valuable feedback and reflections from participants in all our workshops. These feedback fuel our commitment to improving SIC workshops and the broader Serious Illness Conversation Program (SICP). 💬 One of our participants, a senior social worker from Dementia Singapore shared "I had a follow-up call with one of our caregivers of persons living with dementia. To start the conversation, I picked out two questions from the SICG – to ascertain her understanding of her husband’s condition, as well as her current priorities and concerns. Not only was I able to normalise these concerns but the process also gave me the opportunity to discuss end-of-life preparations, without the risk of being too obvious or forward with broaching the subject.” 📄During the workshop, participants engaged in scripted role plays where they practiced conducting serious illness conversations using the locally adapted Serious Illness Conversations Guide (SICG). The participant's sharing reflects the ability to seamlessly integrate the SICG into one's practice to assess the understanding and information preferences of patients. It's heartening to hear feedback from participants that the workshop has normalized serious illness conversations. 🎉 Thanks to all for being part of this transformative journey! ⚡ Drs. New research shows that only 8% of hospitals in the U.S. have instituted all the key components of evidence-based, best-practice interventions that they are expected to conduct to help at-risk patients who are discharged after receiving care. Glad to see strong research in this area so that we can move closer to making sure anyone at risk gets evidence based and high quality care no matter where they seek help. #zerosuicide Zero Suicide Institute
