Cornea Update: Mediators of Visual Acuity in Descemet Membrane Endothelial Keratoplasty and Ultrathin Descemet Stripping Automated Endothelial Keratoplasty: Purpose: The aim of this study was to investigate mediators of visual acuity in ultrathin Descemet stripping automated endothelial keratoplasty (UT-DSAEK) compared with Descemet membrane endothelial keratoplasty (DMEK). Methods: This was a prespecified secondary analysis of the Descemet Endothelial Thickness Comparison Trial, a prospective, randomized controlled trial comparing UT-DSAEK with DMEK. Subjects with Fuchs endothelial dystrophy or pseudophakic bullous keratopathy presenting to 2 academic centers were enrolled and randomized to either UT-DSAEK (n = 25 eyes) or DMEK (n = 25 eyes). Higher order aberrations (HOAs) and corneal densitometry were measured with Pentacam Scheimpflug imaging at 3, 6, 12, and 24 months. Recently we performed a unique test called inferior petrosal sinus sampling (IPSS), for a young girl with Cushing syndrome. When the pituitary lesion is not detected on MRI, it necessitates IPS sampling since the microadenomas that secrete ACTH may be too small to be detected with MRI. It helps us figure out if the extra ACTH hormone causing Cushing syndrome is coming from the pituitary gland or somewhere else in the body. This test is crucial for getting the right diagnosis and treatment plan. This test is pretty cool because it's minimally invasive but super accurate. During the test, we carefully guide a tiny tube into veins near the base of the skull. Then, we collect blood samples from those veins and compare them to samples from elsewhere in the body. It's like detective work for hormones. The results revealed that our patient has a corticotroph adenoma on right side of the pituitary gland. Now, armed with this info, we can move forward with treatment options tailored to her condition. #Neuroradiology #interventionalradiology #Cushing #ipss #endocrine #pituitary
