A very comprehensive investigation & report. The summary video is well worth a 20 min viewing. IMO All the usual traps appear as though they were unpredictable however with the power of hindsight could have been anticipated if the basics were always followed thoroughly (ETTO Efficiency Thoroughness Trade Off): 1. Avoid Common mode failure, 2. Avoid “eggs in one basket”, 3. Avoid modifications/temporary measures on the run, 4. Maintain redundancy 5. Protecting operation output over protecting vulnerability. IMO Once this failure was set up, shut down of the grid became unavoidable but the loss of the plant & danger to personnel could have been avoided if the whole C4 plant was isolated from the grid shutting down all power stations. A BIG emergency shutdown decision difficult to make. IMO This event has similarity with other disasters however by following emergency procedures, loss of life was avoided. Well done to the plant operators in seemingly impossible circumstances.

Safety checks are so important, it gives assurance that all will be good. Most part of my early career days were of quality assurance. My observation was people didn't like Quality checks and wanted the acceptance of product and services. Its due to human nature that we think questioning as our rejection. however, questioning is for quality improvement. Errors are meant to happen during creation, because even perfection is not perfect. The drawbacks of not doing quality check are, the day things go bad a very heavy price has to be paid. Governance and assurance are both vital parts of creativity. Governance works on studying the impact and implementing good solutions and quality ensures it will be done rightly and correctly. The meaning of words defines the purpose.
