Always a debate whether to fix humeral shaft fractures with an intramedullary nail (IMN) or plate fixation (ORIF)? The Level I evidence in the literature does not show a significant difference in rates of union, reoperation, or radial nerve palsy between IMN and ORIF for humeral shaft fractures. Overall, treatment with IMN results in a lower infection rate, less operative time, and a modestly quicker time to union. "So, what do you do for a living?" "Oh, I work in sterile processing." "What is that?!" How often have we had that conversation with friends, family, and neighbors? Sometimes we have an answer ready (especially if we get this question a lot), but other times we stop to think about it. What is sterile processing? This answer could be simple: "You know when a person has surgery, and they put those metal tools inside them? We clean and sterilize them so they're safe." Or the answer could be, well, a short novel covering the department's different areas, patient safety, infection prevention, and a little microbiology to really send it home. Debris within the bladder is commonly seen on ultrasound. The term debris refers to mobile, echogenic particles seen in urine within the upper tracts or bladder. Because of the wide range of etiologies, the finding of bladder debris on ultrasound is non-specific and can be difficult to interpret clinically. Bladder debris in particular is observed commonly, and is often attributed to infection.
