Main takeaways- these are true partners, we need to embrace the EPCC partnership and treat them like cluster partners. Learn their buisness, meet often, share best practices, assist with opportunities and collaborate on new and innovative ideas on how to improve our care continuums, but most of all….tell the story. We have had significant growth in both Skilled nursing and HH Hospice we need to ensure our new employees know this relationship! So….tell the story!

Easier for Family and Friends to Visit: Loved ones can provide emotional support and companionship more easily. Freedom and Independence: Patients maintain independence and autonomy while receiving necessary care. Cost-Effective: Home health care is often more affordable than inpatient care at hospitals or nursing facilities. Remember, the goal is to enable individuals to age in place independently and comfortably.
