I really like The 1% Rule Paul Deighton it's a great concept to show the power of steady improvements over a sustained period of time. I do feel that for most maintenance organisations the priority should be to 'steady the ship' so to speak and eliminate the reactive environment they're in, because in reactive environments it's near impossible to sustain that 1% rule over the 2,3 or 5 years that is required to really make a dent in the defects.
