They focused on improving assessments, accurate diagnoses, and enhancing care. Wow. It has been a long time since I posted anything, not because nothing has been happening...rather because of too much going on. Our clients these days are motivated and undertaking whole floor makeovers - starting with the front entrance. Have a look at how much a door wrap transferred this hospital's ALC unit's (often a long term home for patients with chronic conditions) entrance into a warmer invitation to enter. This is just the start of many changes within. The reverse side of the door is an "exit diversion" designed to reduce a patient/resident's desire to exit. All credit to our client for her wonderful vision of her patient's living space. The Serious Illness Messaging Toolkit was created under a grant generously funded by The John A. Hartford Foundation and Cambia Health Foundation. Regarding advance care planning (ACP), their website states, “For the past 30 years we've messaged advance care as if people don't know what it is. But they do know--and that's why they're not doing it.” Their materials are available to all via open-source licensing and backed by evidence-based market research providing clear, concise implementation strategies for improving how we talk about serious illness.
