If you’ve never met e-patient Dave, you’re in for a treat. His personal story is a perfect example of the importance of patient engagement, effective doctor-patient relationships, and compassionate respectful communication. I’m inviting my entire network to attend this webcast where he will discuss how #HumanUnderstanding is improving experiences on both sides of the stethoscope!
Human Understanding! It's the only sane foundation for achieving real care for people in need. Join me on November 2 at 1:00 ET for a live “fireside chat” webcast with one of my most successful speaking clients ever, NRC Health. Register here: https://lnkd.in/efmXvfcu. [This is a repost with updated graphic.] The subject is their mission, “Humanize Healthcare,” so you can see why I’m thrilled to partner with them! I did a spectacularly successful keynote for them at their 2022 Health Symposium in Seattle. They’re about what’s important to me: helping healthcare achieve its potential, through well-informed partnerships between patients and their providers. Healthcare today has immense pressures, and I love how their approach is to make patient profiles *easily accessible* to each provider, for care that’s truly more individualized… more caring. I hope you’ll join us to talk about how best care (in the context of the *individual family’s life*) produces more happiness with less effort. WATCH: Dr. Vicenta Gaspar-Yoo, our senior vice president of care continuum and transitions, discusses importance of care coordination and the shift to risk-based models. The 12-minute video highlights the following: 🟢 Finding the right post-acute placement for each patient is not a one-size-fits-all approach. The right site can lower cost of care and improve health outcomes. 🟢 Dr. Gaspar-Yoo is focused on improving throughput, care transitions, care management and coordination across AHN. Close collaboration with Helion and its high-performing post-acute care networks is a key component of this strategy. 🟢 Transitioning to fully risk-based arrangements makes operational excellence and patient-centered care more important than ever. 🟢 In 2024, Dr. Gaspar-Yoo is most excited about a new level of blended care coordination for the enterprise across case management, acute, post-acute and ambulatory settings.

What an impact! In a recent publication, Alfred Health determined the use of our Virtual Care Platform resulted in “Substantial hospital avoidance" during COVID-19 as they managed over 175,000 patients. More importantly, the report highlights the need for "an integrated health record across partner organisations as an enabler of success for such future models of care". Vertically integrated and coordinated care must be part of the future Victorian Department of Health's digital health vision. https://lnkd.in/g8fph_56
