“We know that activated patients have better health outcomes, so it is no surprise that those with higher activation levels are significantly more likely to have discussed kidney health with their provider. Not only that, but activated patients with diabetes or hypertension are also more aware of the link between their condition and kidney health.” In a recently published MedCity News opinion piece, Phreesia’s Chief Clinical Officer Hilary Hatch, Ph.D. discusses the importance of patient activation and patient-provider conversations in helping patients manage their kidney health. Every year, approximately 130,000 Americans progress to end-stage renal disease (ESRD). A majority of them and unprepared and “crash” into dialysis, with only 30% to 40% having seen a nephrologist in the last twelve (12) months. Unplanned dialysis starts can seriously impact patients physically and emotionally, leading to depression, anxiety, and other issues. That’s why Healthmap helps coordinate care with nephrologists to ensure an optimal start to dialysis for members in our Kidney Population Health Management Program. Our new White Paper offers success stories from Healthmap patients with analysis from our senior medical leadership. You’ll find it here: https://lnkd.in/gcN2wrrf

🌟 USPSTF reviewing CKD recommendations 🌟 The USPSTF is currently reviewing its recommendations for screening for chronic kidney disease (CKD), which have not been updated since being retired in 2016. This review comes at a crucial time as CKD remains a significant public health issue that affects millions. This is a pressing need. The absence of updated guidance has prompted a bipartisan group of lawmakers to urge the USPSTF to provide stronger and more current recommendations. These lawmakers are advocating for a comprehensive strategy that emphasizes early detection and management of CKD, aiming to improve patient outcomes and address disparities in healthcare access and quality. According to the USPSTF, public comments are closed during the Final Research Plan phase, but there will be another opportunity for comment later during the Draft Recommendation phase. What is your take on this topic? Any ideas? You can read more about it at https://buff.ly/3XaL5Ho #USPSTF #CKD
