At Symbiosis Hospital, we recently achieved a remarkable milestone - the successful TAVI procedure on a patient weighing just 27 kg. This extraordinary case presented several clinical challenges, including extreme low weight/frailty, severe kyphosis, scoliosis, and a bent main artery caused by these conditions. Our expert team navigated these complexities with precision and care. Key to the procedure was the Right Valve Selection: Acurate neo 2 (AN2) (Boston Scientific), chosen for its superior trackability. We meticulously avoided aorta damage, ensuring a successful outcome despite the formidable hurdles. A testament to the efficacy of TAVI, the patient underwent the procedure on a Saturday and was walking home on Monday. This case showcases the remarkable capabilities of minimally invasive cardiac care. We extend our gratitude to our dedicated team: • Dr. Ankur Phatarpekar (Interventional Cardiologist) • Dr. Meghav Shah (Interventional Cardiologist) • Dr. Gourish Shinde (Cardiac Anaesthetist) Their expertise and commitment to providing the highest level of care made this success possible. #SymbiosisHospital #TAVI #CardiacCare #MedicalInnovation #PrecisionMedicine #HealthcareExcellence #Cardiology #MinimallyInvasive #acurateneo2

The Clinical Services In IR Task Force developed a three-stage system for clinical services infrastructure development. This system shall help create awareness of the fact that different levels of service provision can be offered and worked towards. IRs may also use this system to benchmark themselves and set personal goals for future developments in their centre and practice. The three levels of clinical services infrastructure are split into #inpatient and #outpatient. What is your current level? Where do you see yourself in three years from now? What helps you or holds you back? Leave a comment to share your experience! Exploring Cardiogenic Shock Treatment with Multi-Device Simulation #ComputationalLife In our latest breakthrough, we've digitally replicated a patient in the throes of cardiogenic shock to assess the efficacy of four distinct medical devices. Our visual graphs do more than just depict left ventricular volumes during a cardiac cycle – they provide critical insights that guide physicians towards the most impactful treatment options. The evidence is clear and corroborated by literature: · pVADs effectively reduce End-Diastolic Volume (EDV) without significantly affecting End-Systolic Volume (ESV). · IABP optimally decreases both EDV and ESV, improving cardiac function. · LVADs, with their unique triangular volume profiles, substantially reduce the overall volume in the left ventricle. · ECMO, specifically from the right atrium to the femoral artery, increases both ESV and EDV. These simulations are more than data – they are the future of personalized patient care, offering a window into the real-time impact of life-saving interventions. Delve into the intricacies of our pioneering research and be part of the conversation shaping the future of cardiac care. Hot off the press!!! 🕹 #Radiation dose in #endovascular #aortic repair, an important topic 🏒 This study dives into the world of radiation dose of (complex) #EVAR and #TEVAR. The aim is to establish diagnostic reference levels (DRLs) for these interventions. DRLs serve as benchmarks to guide centers in assessing whether their radiation doses are high and if adjustments are needed to ensure safety. The study covers various factors influencing doses, including the type of intervention, equipment used, and more. Data from 3712 procedures across 24 centers were analyzed, revealing significant variations in radiation doses. The proposed DRL for KAP was 162 Gy cm2, 175 Gy cm2 and 266 Gy cm2 for standard EVAR, TEVAR and BEVAR/FEVAR, respectively. This study shows that endovascular aortic procedures can be associated with significant radiation doses and that there are large inter-centre dose variations. Developing DRLs pertinent to those procedures is essential to optimise the radiation risks to patients and staff while maintaining the highest standard of patient care. OCCUPI, an Investigator Initiated Study led by Dr BK Kim (Severance Hospital, S Korea) was presented as a late-breaker at just concluded ESC. The study which was also simultaneously published in prestigious Lancet demonstrated that OCT-guided PCI significantly improves clinical outcomes and enhances safety and effectiveness of coronary stenting compared to conventional angiography-guided PCI in complex cases. The data was presented just a couple of days after the revision of ESC guidelines for the treatment of CCS, which revised the Intravascular Imaging (OCT/IVUS for complex lesion) to the highest possible level IA. The results from OCCUPI and the new recommendations will likely change the practice of PCI with DES for complex coronary lesions by providing stronger support for the superior benefits of OCT guidance over conventional angio-guided PCI. Many congratulations to the Investigators of OCCUPI for running this large RCT.. #OCT #ComplexPCI #DES #ESC2024 #InvestigatorInitiatedStudy #Abbottproud
