“HEALTHCARE MALLS” HOSPITALS OF THE FUTURE Healthcare in India is going through a phase of Revolution. Transcontinental Medical Tourism, particularly towards India is leading to serious competition amongst Healthcare Providers. There is clamor to lure patients by innovations, use of technology and automation, much to the delight of the beneficiaries. Knowledge of Medicine and Medical Technology is fast expanding beyond human cognition. So is the Patient and his expectations. The Concept of future Hospitals needs to change too. Maybe in the lines of shopping malls. All patient related services under one roof. 150-250 bedded Hospital, sufficient parking, Integrated Pharmacies (OP and IP), Appliance and Device stores, Modern Diagnostics, Laboratories, Affordable guest rooms and dormitories for attendants, Eateries, Coffee shops, Shopping centers, Travel agents, Transport providers, ATMs, Police Helpdesk, Media rooms, Conference facilities, Prayer rooms, Insurance agents and so on. List of conveniences that can be provided is left to one’s imagination and can be customized. Costs must be affordable, attractive and sustainable. Dear Friends, the “MEDICAL MALLS” have arrived. Some bright Entrepreneur needs to buy this idea. Investment should not be a problem. What are we waiting for? This idea is generated from the vison of our Prime Minister. This hinders the growth of urban hospitals (it’s shocking!) Hospital chains in tier 1 cities have lately dealt with shooting real estate costs and lack of space. Add to that the ever-increasing traffic, which increases patient travel time and reduces their willingness to make hospital visits. Results? Reduced revenue. Healthcare sector is growing at CAGR of ~22% in last five years as per some report. Hospital industry represents 80% of healthcare sector as per data. It is still expected to grow in double digit in next five years. The capital expenditure and acquisition is at peak as of now. As you are aware , Max is coming up with expansion plan for 4200 beds @ cost of Rs. 5,500 cr. Per bed cost is Rs. 1.3 cr . Apart from Delhi, now the company is eying UP . Some numbers are - investment of Rs.2500 cr for 500 beds in Lucknow including investment in land bank for future expansion. Not sure the breakdown but over all Rs. 5 cr per bed seems high - Rs. 150-200 cr for addition of 265 beds in Sahara ( ~Rs.0.6 cr /bed) over and above initial acquisition cost Max is cash rich, baalnce sheet size ~ Rs. 10,000 cr. ARPOB, EBIDTA, RoCE etc are generally above industry average, mainly driven by higher bed strength in metro as per their investor presentation. They don’t use intelligent transportation system ITS or a GPS tracking software to determine the position of their workers in the field. If you think there is a control center somewhere monitoring operations, you are in for a surprise! No cloud based supply chain technology or management gurus either to drive the business. Their error rate is an astounding 1 in 6 million deliveries, well above the qualifying mark for “six sigma certification”. They are the “Dabbawallas” who provide home cooked lunch to the hungry office goers in Mumbai. Way back in 1890, Mahadeo Havaji saw an opportunity for a lunch delivering service in Mumbai. A Parsee banker wanted home cooked food delivered to his office & Havaji took up the responsibility. He became the first Dabbawalla to introduce a new line of business. Today 5000 Dabbawallas dressed in immaculate white with the trademark Gandhi cap, crisscross western, central & harbour lines and deliver food to 200000 customers within a radius of 150 -200 km. How does the end - end process work ? The tiffin boxes are picked up from houses/restaurants and delivered to the station. A color code on the box reveals details about the offloading station, badge number of the local Dabbawala along with the building and floor number of the receiver. Food is delivered on time. Post lunch, the tiffin boxes are collected and delivered back to the owner. The Dabbawallas use trains, cycles and pushcarts for their service which starts at 9AM and ends at 5PM. They have been around for more than a century, the original backbone of Mumbai’s food delivery system. The men in white have fired the imagination of professors from Harvard to Ahmedabad, trying to explain the model of service excellence without the use of technology. ‘Speed, precision and unfailing commitment are the defining features leading to a zero defect delivery system’, observes Raghunath Medge, President Mumbai Dabbawallas. Sadly, the pandemic dealt a body blow to the Dabbawallas as employees started working from home. Some went back to their villages while others joined hotels to keep the home fires burning.
