Adaptive trials are becoming more and more common in clinical research, especially for figuring out which patients will respond best to new drugs before they hit the market. Women, for instance, make up over half the population but have often been left out of many trials, making it tough to predict how they'll react to different treatments and doses. One great example from this piece is the I-SPY 2 Trial, which started back in 2010. It's the longest-running adaptive trial so far and focuses on tailoring treatments for different types of breast cancer based on individual tumor subtypes. Happy National Patient Advocacy Day! Today is an important day to recognize the important work done by patient advocates and their unique role in the transformation of individual and systemic issues in patient care. If you are unfamiliar with the type of work of patient advocates do, this is an excellent example. In this article from Forbes, CEO Rohit Nambisan outlines measures that researchers can take to make clinical research trials more patient-friendly and representative. By addressing barriers to care, research bias, and adopting a patient-centric approach in clinical research, the advocacy Rohit demonstrates in this article aligns with the goals of patient advocacy: to protect and promote the interests of patient populations. While patient advocates’ work can be on an individual, group, or system-wide scale, their efforts both protect the interests of their patients and increase healthcare equity. For example, by improving representation during clinical trials, or by promoting research and awareness for a rarely studied group of diseases. Their work is essential because an improvement in the inclusiveness of the healthcare industry at any level benefits the system as a whole. The more data we have and the more often we serve wider patient populations, the better care we can provide to anyone who walks through hospital doors. I have immense respect for patient advocates and their mission. I urge everyone who is interested in their work to get involved and support them. The PPMD Town Hall on Clinical Trials is underway TODAY, March 4th, during PPMD's 2024 Advocacy Conference in Washington, D.C.! This meeting is a platform for families impacted by Duchenne and Becker to share their experiences and insights on past, present, and future clinical trials. The primary goal is to hear directly from families and gather valuable input to shape PPMD's advocacy efforts, especially in interactions with the FDA on drug development.  AGENDA HIGHLIGHTS: - Parent/Patient Panel: Diverse voices at different ages and stages sharing their clinical trial experiences, discussing challenges, and expressing hopes for outcomes. - Professional Panel on Patient Preference and Clinical Trials: Featuring a representative from FDA CDER and clinicians covering FDA updates, issues in clinical trials, and potential solutions.  Following the meeting, a recap will be shared with the community and learnings will be integrated into PPMD's advocacy strategy moving forward as we interact with federal agencies (including FDA, NIH, DOD, and CDC). Together, we're working to enhance the clinical trial landscape for Duchenne and Becker families and contribute to policy strategies for improved outcomes. #PPMDAdvocacy

Honest diary of a CEO: not afraid to say the things you are thinking Reflections on industry support for patient advocacy groups There has been a lot of discussion recently about the role of the pharmaceutical industry providing financial support for patient advocacy groups, particularly in the rare disease space. With many companies stepping back from supporting patient advocacy groups due to fears of reputational backlash and complaints to the PMPCA, anecdotally smaller patient advocacy groups are struggling to support their patient communities and campaign for access to new treatments or new service provision. So, is it time to rethink the way support for advocacy groups works and take a more pragmatic approach? Often rare disease advocacy groups are small, but mighty. Working with limited time and funding and sometimes the lack of technical expertise to produce the data and documentation required to support the patient voice during the drug approval process. The amount of work to document the impact of new treatments on quality of life, the impact on mental health and the wider impact on carers is vast.  Not only that, the pressure on a few people to be the voice of an entire patient community and campaign for access, whilst often working and caring as well, can often be overwhelming. Yes, there needs to be consideration about pharmaceutical funding, particularly in rare diseases, where often there is only one company and one treatment. BUT patient advocacy groups should have the option of accepting support if they feel it is appropriate and that choice should be theirs. With industry pulling back, realistically what are the other options? The guidance is unclear and is interpreted by each legal review team in each pharmaceutical company differently, with some taking a black and white approach and others a more pragmatic approach. So, the paying field is not even – it all depends on who you talk to. Maybe it is time to think about a cross industry and patient advocacy coalition to provide multi-funded resources to support advocacy groups through the drug approval and access process through education and training programmes?
