Interesting..was just discussing these topics. Wondering if we will see a need in re-tirations for those not on autopaps, or maybe even some eliminating the need for pap therapy.

With the publication of SURMOUNT-OSA in NEJM, it seems there is a lot of catastrophic thinking that OSA is going to now be eliminated and medical devices used to treat OSA are going to have less patients to treat. I’ve been contacted by many industry representatives regarding these results and mind you I served on the advisory committee for Tirzepatide + OSA with Lilly and these are my comments / thoughts / responses to all this catastrophic thinking: 1) Oh come on, stop being naive! 2) Weight loss, even via bariatric surgery, has infrequently been shown to result in complete resolution of OSA. 3) I lost 40 lbs 2 years ago and still have to use PAP therapy with BMI of ~25 kg/m2. And I diagnosed myself with OSA with my current BMI in 2013. 4) Weight loss augments existing therapies to potentially be more effective with the ability to a) use less pressure with PAP therapy, b) use less amplitude with hypoglossal nerve stimulation therapy, c) potentially transition to oral appliance therapy or positional therapy, and d) high five those lucky patients that actually get complete resolution of their OSA. 5) The field of sleep medicine and the management of OSA is FINALLY getting to a level of being able to offer individualized treatment options for our OSA patients rather than only having a handful of options. 6) Embrace this new option to treat this life-threatening chronic disease. The field of sleep medicine is becoming more and more sophisticated in its evaluation and management of patients!

Where stigma dehumanizes people, medicine centers them. Pop culture and media attention present #chronic #conditions and the people who live with them as homogeneous. Depression, obesity, sleep apnea, HIV, Crohn's, and on and on... #stigma becomes one with the condition. Treatments are whispered about, hidden, pitied or shamed or avoided entirely. But medicine tells people that treatment of their condition - and, critically, their experience living with that condition - is worth close inspection. Clinical research is there, cheering loudly from behind the bench or within a protocol or in a new medical device: this condition is real! It is worth looking at head-on! It is worth time and investment and attention. It is worth improving. You are not alone. You are not this disease. It is exciting to watch medicine change. It is exciting to have multiple treatment options where once there was only stigma. Where once there was only shame. Responsible medicine almost never says "Here is your one solution." Instead, it says, "Let's see what will work for YOU." Where stigma dehumanizes people, medicine centers them.
