I have been a gun owner my entire adult life. I have training provided by the United States Navy and I maintain my skills and my weapons myself.
There is absolutely no question that these are weapons and I own them with defense in mind. I wish I did not feel a need for them.
I would voluntarily register, license, and validate competency in a heartbeat and cannot understand why a responsible citizenry wouldn’t proudly show their capability for ownership rather than allow undereducated, mentally ill or psychiatrically evil equal unbridled access.
I submit to licensure for driving cars, racing cars, operating boats, flying planes, scuba diving. I just don’t get it!
Responsible registration for the ‘right to bear arms’ should be a core requirement to the right.

If you want a moral equivalent, do the same thought exercise regarding speech, religion and assembly.
I certainly do appreciate your sentiment and totally realize your heart is in the right place. Driving/racing cars, flying planes, scuba diving are all privileges. Second amendment of the constitution is a RIGHT to bear arms. Huge difference. What’s next? A license for free speech? The Biden administration has already set records for censorship and violations of the first amendment. As much as I respect what you’re saying, giving up our rights as Americans is not the answer.
They are required to be registered in the Commonwealth of Massachusetts, with vigorous background checks, at 21 years of age. And there is access to emergency mental health care, provided someone petitions the court… just depends on where in this country you live. And there hasn’t been any school shootings here. (Crossing fingers!!) coincidence? Maybe. Maybe not.
The part that hit home the most was acknowledging they were weapons you wish you did not need, but you do. This does create quarrel in the evershifting to bear or not to bear conundrum. I see two only real options here. Change ones mindset or location. Be well.

49 year old presenting after a severe mva vs. tractor trailer rollover with severe progressive neck pain and a mixed c5, c6 radiculopathy.
Minimum movement at 56 67 on flex ext films.
Dexa scans were normal and bone quality was great intra op.
I prefer anterior repairs with posterior support only based on questionable clinical healing and quality. Very rarely have I had to add the posterior component of the repair, in the absence of osteopenia and noncompliance.
I did this out patient yesterday. Part of the reduction segmentally was achieved by gradual screw advance segment by segment, screw by screw, and the c6 anchor clearly helped. I would have used it even in the absence of c6 radiculopathy.
Obviously, allograft vs, titanium vs. peek, my results have always been high with this old standard, so this is one of the procedures where adding me too products hasn’t been attractive to me, but obviously welcome any experience.
I have struggled with plateless interbody implants in deformity reduction and abandoned them a long time ago, 15+ years.

I have pushed back and pivoted for 25 years from level 1 Chief of neurosurgery, to no brain surgery and founding a niche spine center of excellence, to building my first spine ASC in 2012 and realizing we delivered a very intimate, personalized quality of care.
Every one of those steps took me out of the safety net of big funding, institutional delivery of resources, money, benefits and our commodity, humans needing care.
Now, I am 100% outpatient, dropped all ASC insurers but blue cross and Cigna, dropped Medicare, negotiate fair pricing for cash or out of network, and premium bundled payment participation plans. I absolutely recognize that per unit of work I make 80% less than 2010…but because I became more independent, I built financial stability, control and community influence by offering a higher level of service and personalization in a facility and a system that I wholly owned. I have a very direct and personal relationship with a community that is my ‘community’ and I am theirs.
What I worry about in your message is what I have seen every time someone comes looking to pick up my pieces, as I transition to medtech. Several, brilliant young surgeons have come, with enthusiasm, passion, idealism, but ultimately, with their hands out negotiating what the hospitals would pay them, talking lofty guaranteed salary, and commitment of patients, no community, no “sweat”.
To get your beautiful fantasy world, that I believe I found a form of, you have to be able to say to your wife, “Hi, how was your day. I got in a fight with the CEO of the hospital today over quality. Yeah, I resigned from Chief role and quit brain surgery, how was your day?”
“What am I going to do, easy, I am going to go to a small low profitable hospital in town, with zero distinction, and build a spine center of excellence, first one in the state and bring people from all over the country”.
And her reply was ok, and then, when that hospital became the most profitable in the metro area and was acquired by the previous system, time to build an ASC with the skills and business tools we learned creating a spine center of excellence, JCAHO certified.
Constant evolution, growth, stress, creates the diamond that people assume was easy. I was lucky my wife rolled with mad fluctuations of income.
My ethos was, “what could go wrong, I am a biomechanical engineer and a freaking neurosurgeon, we will be ok”.
The problem is we set our own shackles, when we lease our career, rather than own. This is what every employed surgeon does! Some land in fabulous work place environments with tremendous joy, many end up miserably unhappy and impotent. They own nothing but what they take when they walk out with their box and what’s inside of them.
To me this is the problem with the modern doctor and loss of power.

52 year old aerospace engineer presented with upper extremity weakness in biceps and triceps on right and triceps on left, numbness and severe cervico-brachial pain.
This patient was referred to me by a very conscientious colleague who didn’t feel comfortable with Arthroplasty for this degree of degen and the patient was motivated for mobility.
C56 right was severe, with complete closure of the foramen and required a lateral foramenal dissection and substantial removal of lateral calcified disc osteophyte complex.
We did this outpatient with complete improvement of all neurological symptoms and objective findings of gross normal strength compared to Preop which was 4/5.
Level 1 soft disc herniations and cervical tdr is straight forward but a very small percentage of people presenting to a spine surgical practice.
I definitely encourage a more advanced mentorship with experienced Arthroplasty surgeons to expand your capabilities for your future patients. The majority of this market is compatible with motion preservation procedures.
