Healthcare teams in Daajing Giids and Masset have created a virtual support network that is helping to ensure patient safety on Haida Gwaii. The virtual network was inspired by the Real-Time Virtual Support (RTVS) Most Responsible Provider (MRP) model. Since 2021, providers on the Emergency Medicine pathway – the Rural Urgent Doctor in-Aid or RUDi pathway – have supported 12 rural emergency departments across BC. What’s holding up our ED’s? It’s not the wrong patient. The article from InSight+ by Caitlin Wright challenges the common belief that many emergency department (ED) patients could be more appropriately treated by general practitioners (GPs). It focuses on a study analysing data from Nepean Hospital's ED, which found that a significant proportion of patients deemed suitable for GP care were actually unsuitable due to various complexities. The study questions the use of Australian Institute of Health and Welfare criteria for identifying GP-type patients and argues against using this definition for health policy or resource allocation. Additionally, the article highlights systemic issues in healthcare, like the state-federal divide in Australia, impacting ED overcrowding and service delivery. Thanks for sharing Andy Jones . Very few of the 250+ patients in my ED could have been managed by a GP. or at least not without access to Hospital in the home services or the ability to refer patients for a inpatient team review without coning to ED. if outpatient specialist appointments were available within a week or less, GP could genuinely manage acute on chronic issues. But the way the system is currently functioning, any issue that needs a specialist review sooner than 3 months away gets a trip to ED. The solution to ED overcrowding is not more GPs, but alternate pathways to specialist review, and reduced waitlists for NDIS and aged care placements to avoid long term chronic patients blocking up ward beds.

Until the states and federal governments actually work together, with clinicians - there will be no useful solutions - all we are seeing is piecemeal, tokenistic initiatives by governments who want to be seen to be doing something to fix bed block without actually fixing it. Examples: Ambulance beds replacing short stay beds (just moves the problem inside, out of the view of the press) Urgent Care Centres - funding what could be done in general practice and servicing patients who don’t need admission and therefore don’t contribute to bed block More ambulances - just makes for longer queues To address bed block one needs to get down to the fundamental basics of patient flow - if you operate hospitals at >95% occupancy , if you slow discharges by restricting suitable locations (eg suitable supported accommodation, Aged Care places) and if you operate most of the hospital on a Monday to Friday, 9-5pm basis - then you cannot achieve flow…. There are a lot of contributors, a lot of stakeholders and it is a wicked problem, but if we can’t even get the basics right, there will be no resolution.

They're basically an extension of the less efficient care #hospitals often provide, and a way to lure patients into inpatient admissions. FSEDs aren’t really about serving communities better—they’re about bringing patients to the hospital instead of more appropriate settings. But hey, in #Florida, there’s a better option—our “high acuity urgent care” model at GEMD. We combine the capabilities of an FSED with the efficiency of #urgentcare. We work with payers and providers to prevent unnecessary admissions and focus on value-based, high-quality care.
