Thank you Chris Koller and Kavita Patel - you describe a plight that requires transformational thinking that goes beyond making the current workforce take on greater workloads. Even our current concepts of team-based care continue to force the PCP to remain singly accountable from a billing and quality measurement perspective. “The portion of dollars that’s going to primary care in the U.S. is declining. It’s declining across all payers. We’re stuck in a payment system for primary care that does not reward team-based care. In terms of access to primary care, what we found is that even though we have increased the portion of people with insurance coverage in the United States to record-high levels, there are fewer and fewer people who report having a usual source of care. Care is fundamentally about a relationship, about a place that we go when we’re not feeling good, when we’re trying to figure out how to navigate the health system. Building in healthcare requires one thing: pain tolerance. Everything will take longer and grow slower than you expect. There will be regulatory and incumbent challenges that threaten your existence. You will lose multiple key deals and even your internal sponsors will sometimes let you down. *** Join 6,000 healthcare readers of my newsletter: www.HealthcareAndy.com *** We tend to glamorize startups in America, especially between 2020-2022 when investor $ for entrepreneurship was falling from the sky like rain. But what I've realized is that ever-lasting healthcare progress is measured in decades, not years. And this is where pain tolerance comes into play. I hate to romanticize hustle culture, because I do believe that people should take care of their physical, mental, and emotional health. But meaningful change requires a lot of hard work. I've talked to some executives / founders who look back at their careers and can literally recall bad months or even bad years on their way to building something special. I wish it were different, but it feels inevitable to some degree. The only thing I can do is focus on a problem that is personally meaningful and commit to spending many years without fail. Burn the boats mentality. I can't guarantee success, but I can guarantee my effort. And hopefully along the way I can attract similar-minded leaders who are committed to building something special too. So... what do you think? Are you in the dream job?
