My patients hate the pushy and repeated calls from Optum / United Health saying that "Dr. Tipton" wanted a nurse to come to their home for a "wellness visit". A lie. Meanwhile I told my patients to block the calls and refuse to accept the visits. "Tell them you have a real doctor who knows me and will see me in person!" I told them. I hate the 13 page useless reports I get from these nurses after the visits. "Check with your doctor" it says about every single thing... Things that I already know about. "Consider a pneumonia vaccine". Oh yeah, how insightful. 🙄 After these home visits I suddenly got papers from the insurance company saying that my patient had "secondary hyperaldosteronism" or "malnutrition" or peripheral artery disease" or "diabetic cataracts". Why these diagnoses? Because certain diagnoses, even when made by a nurse who just met my patient and has poor or no diagnostic capabilities on a home visits, funnel billions of dollars to companies like UnitedHealthcare This article explains how these annoying visits are anything but harmless. Incentives built into the Medicare Advantage programs are being exploited and they will bankrupt Medicare. Forget Medicare for All - we can't afford Medicare for our seniors. Thank you Anna Mathews and team for the evidence that "Sixty percent of UnitedHealth home visits generated at least one new revenue-producing diagnosis of a condition no doctor was treating, the analysis showed. " 60%? You know what that means... these are NOT real or significant diagnoses. It's a scam.

NHI EXPLAINER PART TWO During the Parliamentary process a couple of constitutional objections were raised over above those I highlighted yesterday. The main constitutional arguments related to freedom of choice, professional autonomy, property rights and right to economic activity. This was balanced against section 27 of the Constitution, which is the right to health. What we have learnt from the Constitutional Court in the Clicks case is that the courts will have more sympathy for government policy considerations over business interests when it relates to progressive realization of health rights enshrined in the Constitution. In addition, the AllPay cases showed that the Constitutional Court is also willing to take lucrative business from companies and hand it back to the State especially where the State has constitutional and legislative obligations to protect and maintain rights of citizens. It is interesting to note that in terms of the NHI Bill, medical services providers can choose to be accredited to the fund and medical schemes are not prohibited from continuing to offer their services. Of particular interest is that Section 33 of NHI Bill states that once NHI is fully implemented, medical schemes may offer complimentary cover to services not covered or reimbursable by the NHI Fund. This is similar to what happens in the UK where you can pay for or seek medical insurance for medical services not provided for by what they call the NHS. It will be key for a complete list of health services covered to be published as soon as possible showing how the phased in approach will work for each service. This will then give a better picture on the implementation of section 33 of the NHI Bill. In a nutshell, the constitutional arguments raised against the NHI Bill will have get over the hurdles I have raised. I suspect that the best course of action to take to challenge this Bill will mainly be on practicality arguments rather. Part 3 will be on Monday. Keen to see some of your views on this.
