See how the lab of your dreams can improve patient outcomes... Daisy, 10-yo FS mini dachshund, presented to her veterinarian for anorexia. Physical examination revealed a mass in her cranial abdomen. An abdominal ultrasound using Heska’s US10 veterinary ultrasound system showed a singular liver mass but, no other masses or enlarged lymph nodes. CBC results from the Element HT5 were normal as were thoracic radiographs captured with Heska's Cuattro Digital Radiography suite. However, chemistry results generated from the Element DC5X revealed mild liver enzyme elevations and slightly low blood glucose. After consultation with their veterinarian, Daisy’s family opted to pursue surgical mass removal. Pre-surgical coagulation testing was performed on the Element COAG+ revealing normal PT and aPTT values. Mass removal was successful. Pathology confirmed a single hepatocellular carcinoma. Daisy received full imaging and laboratory work up within a day putting her on the road to recovery faster than ever! There are three different stages with antifreeze intoxication. In stage 1, which usually occurs within 30 minutes, there is vomiting, increased drinking and urination, as well as central nervous system depression and ataxia. A decrease in body temperature is also a common symptom in this stage of poisoning. In stage 2, typically 8-24 hours after ingestion of ethylene glycol, patients initially show clinical improvement. However, in stage 3, 1-3 days after poisoning, acute kidney failure occurs with little or no urine output. A massive increase in kidney values, significant electrolyte imbalance, and acidosis resulting from acute kidney failure lead to a life-threatening condition. In this phase, hemodialysis is the only curative treatment. Therapy in stage 1 has an excellent prognosis, while the prognosis or survival in stage 3 is considered extremely cautious to poor. Emergency measures at the onset of poisoning include induced vomiting and gastric lavage. Hemodialysis with ethanol can save the patient's life. Feline Orofacial Pain Syndrome (FOPS) is a condition associated with episodes of severe oral discomfort. Pain in FOPS often recurs and is poorly responsive to first-line treatments. FOPS is a neuropathic pain disorder. Similar to human medicine, chronic orofacial pain can be effectively alleviated with minimally invasive Interventional Analgesic Procedures (IAP), especially when oral medications fail to relieve the pain. vtx:think Did you see the case of a 5 year old Maltese on our feed earlier in the week? If not don't worry here is a quick recap: A 5-year-old FN Maltese presents to the clinic with acute vomiting, abdominal discomfort and depression. Radiographs of the abdomen identified a region of heterogeneous loss of peritoneal detail, which is associated with an ill-defined increase in soft tissue opacity (see the previous post for images). What would you conclude from these clinical findings? What would your next steps be for this case? Radiograph findings are concluded as non-specific. The most suggestive of regional peritoneal effusion/inflammation. Given the location and appearance, this is most likely due to acute pancreatitis. diagnosis of GI ulceration, less likely pancreatic or gastrointestinal neoplasia. No evidence of an obstructive pattern, no sign of intestinal dilation, foreign body or mass lesion. Ultrasound examination of the abdomen is also recommended, with focus on the gastrointestinal tract and pancreas, to confirm / rule out any other pathology since radiography is relatively insensitive for pancreatitis. Symptoms of THC intoxication include vomiting, hypersalivation, urinary incontinence, depression, weakness, trembling, ataxia, mydriasis, nystagmus, tachycardia, respiratory depression, coma, and death of the patient. Symptoms can occur up to 36 hours after ingestion. Dogs can become intoxicated with THC if they, for example, ingest hashish, marijuana cookies, or a joint stub. Emergency measures for THC intoxication include decontamination through induced vomiting and gastric lavage. Since THC is highly lipid-soluble, therapy with a lipid-rich infusion solution (Lipofundin) can also be administered initially as a bolus followed by continuous infusion. By increasing blood lipid levels, THC cannot optimally pass through the blood-brain barrier. #pathologychronicles During the Christmas New Year break.... Saturday December 23rd 10:16am - a Doctor emailed demanding that he wants this and that. That he is a large referrer. He can go to any Pathology provider. 10:37am I replied back stating that we receive approximately 1000 cases per day and that all business is not good business. I stated that that we spend alot on quality systems to ensure his demands of reporting requirements are met. Doctor requires specific words eg IEC, Bowen's Disease, SCC insitu etc. Most providers have issues with SCAP and SCALP when written on the request form. Data entry teams type Scapular. Other sites mixed up are SHIN vs CHIN. Our LIS (LinkFinity) detects this and rolls back the report for fixing/checking. If a Doctor prefers the words SCC Insitu. The system checks and rolls back if it is not used. It was quite a heated email exchange. I emailed our BDM and CEO. Stating ... I'm not sure if this Doctor will use our services. Today at 1:54pm the Practice Manager of Doctor called our Office Team. Saying that he wants to talk to me saying that Doctor and myself are stubborn people and he wants to smooth things over to use our services. Pathology providers are not respected for the work we do. #respectpathology We do not have the corporate size to have mixed billing, whilst our clients do. Especially a non corporate like us.
