Oesophageal adenocarcinoma is a cancer of the gullet which is often caused by irritation from stomach acids over time (1). Although this disease is becoming more common in the western world, survival has improved with advances in treatments including chemotherapy, radiotherapy and surgery (1,2). Medical imaging presents an opportunity improve risk assessment at diagnosis, by modelling tumour shape and texture characteristics known as "radiomics" (3). However, many challenges exist in choosing relevant features and quantifying their relationships with patient outcomes. For example, different scanners and settings may produce slightly different feature values (4). To assess the reliability of imaging models in clinical practice, it is essential to test whether model predictions hold true in data from different scanners and hospitals. In this study (5), we modelled computed tomography (CT) textures to improve predictions of life expectancy for patients diagnosed with oesophageal adenocarcinoma in five NHS trusts, demonstrating the value of both radiomics and clinical predictors such as age and cancer stage. 📃Scientific paper: Alternative- and focal therapy trends for prostate cancer: a total population analysis of in-patient treatments in Germany from 2006 to 2019 Abstract: Purpose Focal therapy (FT) offers an alternative approach for prostate cancer (PCa) treatment in selected patients. However, little is known on its actual establishment in health care reality. Patients and methods We defined FT as high-intensity focused ultrasound (HIFU), hyperthermia ablation, cryotherapy, transurethral ultrasound ablation (TULSA) or vascular-targeted photodynamic therapy (VTP) TOOKAD^®. We analyzed the nationwide German hospital billing database for a PCa diagnosis in combination with FT. For analyses on the hospital level, we used the reimbursement.INFO tool based on hospitals’ quality reports. The study period was 2006 to 2019. Results We identified 23,677 cases of FT from 2006 to 2019. Considering all PCa cases with surgery, radiotherapy or FT, the share of FT was stable at 4%. The annual caseload of FT increased to a maximum of 2653 cases in 2008 ( p  < 0.001) and then decreased to 1182 cases in 2014 ( p  < 0.001). Since 2015, the cases of FT remained on a plateau around 1400 cases per year. The share of HIFU was stable at 92–96% from 2006 to 2017 and decreased thereafter to 75% in 2019 ( p  = 0.015). In 2019, VTP-TOOKAD^® increased to 11.5% and TULSA to 6%. In 2006, 21% (62/299) of urological departments performed FT and 20 departments reached > 20 FT procedures. https://lnkd.in/e6Vbz82g Here is the first paper derived from the NTCP models of my medical physics specialization thesis on the side effects of tangential RT treatments in breast cancer. Analyzing over a thousand patients with 48 months of follow-up, we focused on the dose to the first 5 mm of the skin as a potential structure responsible for these late events. Results highlight the impact of volume in cc at 20 Gy (50% of the prescription dose) and 42 Gy (105%) for patients tested without RT boost. However, for those experiencing cosmetic deterioration post-surgery, radiation therapy's impact can be challenging and may lead to a high risk of late symptoms (no surgery effects found in the acute phase). High-intensity focused ultrasound (HIFU) ablation can palliate pain, destroy early-stage tumors, and debulk unresectable cancers, according to a newly published study. The comprehensive review led by Alessandro Napoli, MD, PhD, at Sapienza Università di Roma, summarized all clinical research HIFU applications for tumor ablation, including cancers of the breast, prostate, pancreas, liver, kidney, and bone, as well as non-cancerous tumors like uterine fibroids. TREATMENT THURSDAY The treatment options for bladder cancer largely depend on how advanced the cancer is. Treatments usually differ between early stage, non-muscle-invasive bladder cancer and more advanced muscle-invasive bladder cancer. 𝗠𝘂𝗹𝘁𝗶𝗱𝗶𝘀𝗰𝗶𝗽𝗹𝗶𝗻𝗮𝗿𝘆 𝘁𝗲𝗮𝗺𝘀 (𝗠𝗗𝗧𝘀) All hospitals use MDTs to treat bladder cancer. These are teams of specialists that work together to make decisions about the best way to proceed with your treatment. Members of your MDT may include: - a urologist – a surgeon specialising in treating conditions affecting the urinary tract - a clinical oncologist – a specialist in chemotherapy and radiotherapy - a pathologist – a specialist in diseased tissue - a radiologist – a specialist in detecting disease using imaging techniques You should be given the contact details for a clinical nurse specialist, who will be in contact with all members of your MDT. They’ll be able to answer questions and support you throughout your treatment. Deciding what treatment is best for you can be difficult. Your MDT will make recommendations, but remember that the final decision is yours. Before discussing your treatment options, you may find it useful to write a list of questions to ask your MDT. Your MDT may recommend a specific treatment because of your individual circumstances. For example, someone with a small bladder or many existing urinary symptoms is better suited to surgery. Someone who has a single bladder tumour with normal bladder function is better suited for treatments that preserve the bladder. 📃Scientific paper: Real-world outcomes of patients with renal cell carcinoma, surgically treated at regional hospitals, based on a prospective long-term survey of the pre-robotic era Abstract: Purpose Renal cancer surgery is frequently performed in small regional hospitals in Japan. This study evaluated the outcomes of renal cancer surgery, comparing results from the pre-robotic surgery era with those obtained with robotic surgery. Methods This prospective cohort study was conducted on patients who underwent renal cancer surgery between 2008 and 2013 at 14 hospitals, comprising 13 regional hospitals and a university hospital, registered in the Tohoku Urological Evidence-Based Medicine Study Group. The patients’ backgrounds; perioperative data; annual postoperative renal function; and prognostic surveys, performed over a median follow-up period of 10 years were obtained. Results In 930 surgical cases at the 14 registered hospitals, the 10-year recurrence-free survival rates of cT1a, cT1b, cT2, and cT3 were 0.9326, 0.8501, 0.5786, and 0.5101, respectively. Meanwhile, the 10-year overall survival rates were 0.9612, 0.8662, 0.7505, and 0.7209, respectively. Long-term observation in patients with cT1 showed that vessel involvement and high tumor grade were prognostic factors for recurrence. As a noteworthy fact, radical nephrectomy was performed in 53.3% of patients with cT1a at the regional hospitals. However, even in patients with preoperative chronic kidney disease stage 3, radical nephrectomy was not a prognostic factor of renal function. Great to see that axillary clearance in EBC may be on the way out - see paper below. This paper made me think once more about whether the direction in which the incision for axillary sentinel node biopsy has any effect on the later development of breast #lymphoedema. That is, does a transverse incision across the line of the lymph channel/s increase the potential for development of scar tissue that may create adhesions around the lymph channels, thus reducing lymphatic flow (particularly in people with a propensity for developing excess scar tissue)? Would a longitudinal incision create less of a risk for development of adhesions and development of lymphoedema? Scar tissue (and fibrosis from XRT) can develop over time, so perhaps explaining the later development of lymphoedema in some people? Anyone have any thoughts, or know of any evidence in this regard?
