Inspired by a recent message from a young doctor finishing his fellowship. Times are tough in healthcare and an alarming number of medical students, residents, fellows, and early career docs are looking for a way out (before they even get started). The allure of startups, health tech companies, and clinical roles within Big Tech and Big retail is strong. But the grass isn't always greener, and you don't know medicine until you know medicine. In life, running towards something is usually a better path to success than running away. For medical students / residents / healthcare providers in training who are thinking of a career in digital health, this article is spot on. As someone who has helped build / lead a digital health startup, I can confidently say that if your intention is to bring "clinical value" to a startup, what you bring is only as good as your real clinical experience, not your degree.

Action is the key to driving change. Please share this with any resident/fellow or early career physician you know: In my spare time, for the last year I’ve been working on a project with physician leaders around the country thinking about ways to improve the lives of early career physicians. My part is centered on my belief that all graduating residents and fellows would serve themselves well to get their GCP certification in their senior year of training, and build a brand as a clinical researcher with whatever employer they seek. Is this self-interested? Sure - just like Google benefits from anything that increases the speed of the internet, Circuit Clinical benefits from things that increase the number of available and research-interested physicians (at one millionth the scale of course!) But more broadly, the vast, vast majority of those doctors will pursue research away from our org - and that’s still a win as far as I’m concerned. We need the entire industry interested in this problem - it’s hard, but solveable, and high-impact. One signal this message is resonating with high-adoption physicians? I gave Grand Rounds for Family Medicine residents on the topic of career management and this was the single most in-demand component of the talk. The idea that research might keep medicine intellectually stimulating and prevent burnout has the virtue of “sounding right.” Now we have to put it to the test. Want a younger, more diverse investigator base? We graduate 40,000 new docs every year - they may represent our best hope of scaling research access more equitably in the near future. And they are sending signs they’re looking for the on-ramps. Next up will be making it “easier” for them to get on the highway. #clinicaltrials #physicianburnout

Would someone enlighten me as to how Epic is not considered a MONOPOLY in the area of healthcare Electronic Medical Records? Monopolies drive down competition and drive up prices, they make it harder for newer/smaller companies to break into/succeed in the market and drive down innovation. "In fact, with 231 teaching hospitals under its belt, Epic controlled nearly 60% of the academic medical center market in 2022, KLAS Research reported. Epic was the only EHR vendor to pick up any academic hospitals in 2022, with four (Oracle Cerner, Meditech and Altera Digital Health each lost one). More than 90% of medical students and residents now train on Epic." "Epic's reign, however, raises the "existential question" of whether it's good for EHR competition or healthcare in general, he said. "My colleague said, 'Look, you used to be worried about Skynet, but they're not so dominant," Dr. Sharp said with a laugh, referring to the fictional AI network that threatened humanity in The Terminator. He also pointed to Epic's Cosmos informatics platform, which aggregates data from 217 million patient records for researchers to draw clinical insights from. "That's of great value to an academic organization. Where else can you get on a platform that has 210 million lives of data? This is kind of extraordinary," he said. "The bad news is that it's going to be Epic-centric.
