The key to understanding demand for home care is understanding trends in exits. People tend to focus on the number of people in a home care package since the that reflects the cost to government and the number of clients for providers. Package recipients are up 256% from September 2017 to September 2023. That’s a huge increase. But it doesn’t really reflect growth in demand since there were lots more people on the queue in 2017. My best estimate for the increase in overall demand since 2017 is about 87% (counting current recipients and people waiting with some assumptions to fill data gaps in the older data). That is still a big increase in demand over the 6 years. But the number of HCP approvals is only up 27% over the same period. The apparent discrepancy is explained by the fact that exit rates from HCPs have dropped. As far as I can tell, exits fell from 28% of HCP recipients in 2016/17 to just 17% of HCP recipients in 2022/23. If the exit rate had stayed the same there would have been an extra 35,000 packages cycling through to new recipients in 2022/23, which probably would have eliminated the queue entirely. I suspect that exits from the queue have also slowed, but there isn’t much data on this. Part of the reduction in exits may be that there are lots of new package recipients. But I suspect that newer cohorts of HCP recipients are also likely to stay in home care for longer. I think we have a pretty good handle on approvals, with clear trends in age/sex-based approval rates. This may be influenced a bit by the support at home changes. But understanding exits is really the missing piece to properly understanding demand, which is itself a pre-requisite to a mature conversation about granting demand driven access.

Two topics really stood out to me: 1️⃣ Home care agencies are still brainstorming ways to take on more short-hour shifts.
