Through DPC, Dr. Steelman has taken control. He sees fewer patients, allowing him to build stronger relationships and provide the personalized care he envisioned. Becker's Healthcare: Louisville, Ky.-based Baptist Health Louisville introduced a new model to improve service quality and optimize patient outcomes. The model, called the "tiered performance structure," establishes a framework for recognizing and rewarding exceptional patient experience performance, with a key component of the model being the development of a standard algorithm. Yale New Haven Health recently celebrated a significant milestone, caring for our 1000th patient through the innovative Home Hospital program. Former patients Melissa Desmond and Reverend William McCullough shared their positive experiences, emphasizing the convenience and quality of care received at home. The program, launched in 2022, offers acute care services to patients who would otherwise require hospitalization, utilizing a mix of in-person visits and telehealth technology. “It was so much easier and convenient for me and especially my children to receive great care at home. The biggest takeaway for me was being able to be with my family and have the staff right there to answer all my questions,” said former patient Melissa Desmond.
