In honor of Patient Experience Week, here is a reflection on patient- and family-centered care. In my experience, this term is utilized frequently but not always well understood. The Institute for Patient- and Family-Centered Care (IPFCC) defines it as “an approach to the planning, delivery, and evaluation of health care that is grounded in mutually beneficial partnership among healthcare providers, patients, and families.” IPFCC additionally describes four core concepts of patient- and family-centered care as: -Respect and Dignity: Health care practitioners listen to and honor patient and family perspectives and choices. Patient and family knowledge, values, beliefs and cultural backgrounds are incorporated into the planning and delivery of care. -Information Sharing: Health care practitioners communicate and share complete and unbiased information with patients and families in ways that are affirming and useful. Patients and families receive timely, complete and accurate information in order to effectively participate in care and decision-making. -Participation: Patients and families are encouraged and supported in participating in care and decision-making at the level they choose. -Collaboration: Patients, families, health care practitioners, and health care leaders collaborate in policy and program development, implementation, and evaluation; in facility design; in professional education; and in research; as well as in the delivery of care. Heartland Whole Health Institute is proud to champion a revolutionary paradigm shift towards a Whole Health Approach. Often I hear "Ok great! But what exactly does that entail?" so I want to take this opportunity to explain. A Whole Health Approach centers on placing patients at the core of a collaborative, preventative, and value-based care model. Heartland Whole Health Institute was founded in 2019 by the visionary philanthropist Alice Walton and the institute stands as a beacon of innovation in the healthcare landscape. With a mission to advocate, educate, and guide the implementation of a transformative system, HWHI is dedicated to realigning financial incentives and fostering holistic wellness. I am honored to serve on the board of directors with esteemed individuals like Alice Walton, Toby Cosgrove, Ross DeVol, James Walton, Steve Mackin, Steve Nelson. With President and CEO Walter Harris and the entire team, together we are poised to drive impactful change in the Heartland and beyond. Lifelong Medical Care - Episode Six: Health Investments for Berkeley Lifelong Medical Care's community-centered Heart 2 Heart and Oral Health programs bring critical health services into their communities. Their community health advocates/leaders ensure services are accessible and work in partnership with trusted local organizations to build community agency and empowerment to improve health and well-being. In a special CBPH mini-series, Praxis is featuring the community-led efforts to improve health that have been been funded in part through investments from Berkeley’s soda tax.
