Congratulations Daniel Ogunniyi To receive this Certificate of Award, Daniel had to populate a 130-page plus IQ OQ PQ protocol to qualify a Clean-in-Place System in a pharmaceutical manufacturing plant. The protocol detailed the following elements: - Product characteristics – showing what your system is looking to achieve/produce - Production equipment – detailing the equipment necessary - Test scripts and methods – telling you the steps involved in conducting a test - Test parameters and acceptance criteria – defining acceptable test results - Test checksheets – documenting and recording the test results - Final approval – documenting that the validation process has been successfully carried out. And to complete this assignment, Daniel had to learn the following: - How to prioritise which parts of the equipment system are critical to qualify and which parts are not etc, using component impact assessments and risk management tools. - How to read/interpret the engineering documentation such as P&IDs, Piping Isometrics and Electrical Loop Drawings used to define the equipment system. - How the intent of a process (like maintaining the product at the required temperature) is linked to the equipment that will be used to do this. - The tests along with the functional test scripts necessary to confirm that the installed equipment is fit-for-purpose, i.e. that it will do what it states that it will do based on the User Requirement Specifications). - How to use a traceability matrix tool to ensure that all requirements defined within the User Requirement Specifications (URS) are tested in the protocol. - How to qualify equipment, instruments and piping systems, and automation controls. CVSA & FMCSA Working Towards An Operational Test of In-Motion CMV Inspections (Level VIII Inspections) In collaboration with CVSA, FMCSA’s goal is to enable Level VIII Inspections by working with States and technology vendors to provide the capability to make the electronic transfer of the Level VIII data possible, and by implementing an operational test to measure the impact of adding in-motion inspections as another tool for monitoring motor carrier safety compliance. The Level VIII is not currently in effect, but CVSA and FMCSA are pushing strongly to deploy it across the United States and potentially Canada. Hello again fellow Linkers.I taught a class yesterday on MRM principles and Human Factors.We discussed how to use all available resources to carry out aircraft maintenance tasks efficiently,effectively and safely,to ensure our aircraft are compliant and fit for release to service.Also with Human Factors,we discussed the "dirty dozen" and their mitigating factors or safety defences. Also as part of our discussions, this incident was talked about. How the safety defences failed when the "cheese holes aligned" and thus this outcome. We discussed how an individual's slip,lapse,error or negligence can have ripple effects within the organization,stakeholders and our customers. Some errors can be latent and be systematic in nature and may never manifest itself until an active error exposed them. Our MRM/HF training strives to create a safety mindset and error free performance for our maintenance teams.Always remember "Bad outcomes,like Good outcomes are a team effort".
