Success for ACOs and other value-based care arrangements depends on right-sizing care and keeping the locus of care in the community. According to new research, home-visit based primary care led by advanced health care practitioners offering primary care is very successful at that for the neediest and frailest patients. Such a program at Memorial Care Medical Group in California decreased costs by 21.4% while reducing inpatient and emergency department visits as well as 30-day readmissions. A great model to follow. #CareCoordination can bring a lot of benefits by driving alignment among different groups. Assessment of a new model shows that #patients had improved access to #primarycare when their care has a broader arc. That finding may alleviate some of the negative aspects of the data flowing from the model, namely that #hospital readmissions were not necessarily decreased as much as predicted. The varying data points suggest that #valuebasedcare style models can take time to evolve and may not result in the ending that was expected ahead of time.
