When exploring value-based health care implementation one of the major barriers often identified by services and organisations is the inflexibility of current payment models, such as fee-for-service, in allowing for the innovation and collaboration necessary to drive value. As a result, public and private payers around the globe are increasingly pursuing alternative payment models that give providers incentives to improve value. The rationale is that providers and organisations are in the best position to identify ways to reduce waste and overuse, coordinate care across settings, steer patients to the most appropriate, high-quality providers, and provide the care that is needed. One alternative payment model gaining traction globally is bundled payments as they capitalise on the provider entity’s need to manage a budget and ensure quality. The entity receiving the bundled payment earns a higher margin if a patient utilises less care, but also must cover the cost of unexpected utilisation and complications. A 2020 study by the Commonwealth Fund sought to describe the key design elements of bundled-payment models and evaluate empirical evidence about their impact on quality of care and medical spending. 23 bundled-payment initiatives across eight high income countries were identified with results indicating that bundled payments have had a predominantly positive effect on medical spending and quality of care. Private equity investments in healthcare are part of the cause for short-term thinking - looking to gain quick profits that often cause burn-out and impact patient care. Infinite thinking takes a longer-term focus that puts patient care and outcomes over short-term profits. Interesting how infinite thinking and proper planning aligns with value care models, which focuses more on improving outcomes and lives for patients and not as much on short-term profits.
