Working Wednesday Tip Did you know? Cardioversion is a cardiology procedure to restore the heart's normal rhythm. The goal is to shock the heart back into normal rhythm. The associated arrhythmia (irregular heartbeat) is usually atrial fibrillation. Cardioversion is done in a scheduled (elective) outpatient setting in a Cardiology Department. A sedative medication is given to help the person be more relaxed and pain-free. During the procedure, the person is connected to a defibrillator machine. Clinical Pearls/Standard of Care: 1. Obtain an EKG before and after the Cardioversion procedure. 2. Start blood thinners (as indicated by a Cardiology provider). 3. Complete an echocardiogram (sonogram of the heart) to ensure no blood clots are in the heart chambers. 4. Inform Consent with an explanation of the risks and benefits. 5. The sync button should be pressed before EACH shock is delivered. 6. Competency/training with the defibrillator machine to avoid lethal cardiac arrest (R on T phenomenon) ventricular arrhythmia. Is rehabilitation/physiotherapy a necessity or a need? Can one get fine without physiotherapy? The answer is yes / no. To get optimal recovery from a physical deliberating condition like stroke, arthritis, mention it, one should include rehabilitation in the treatment plan. However, most health professionals don't include it in their plans because; 1. Lack of adequate knowledge on how rehabilitation can improve the patient's condition. It is absurd that most of the primary health professionals who shed be teaching and encouraging patients to go for rehabilitation don't know what rehabilitation professionals do 2. The high cost of rehabilitation in most countries. Imagine a single visit to a physiotherapist to help you with a chronic condition in Uganda averagely costs UG shs 60,000/=($16). That means if you have to adhere to the the clinical guidelines of attending a rehabilitation session daily for a stroke patient, you are expected to spend 1.8m Ug shs (480$) in a month. That's minus the transport costs, feeding, other medications. Which average person can afford that? 3. The most important point why people don't include rehabilitation in the treatment plans is the lack of interdisciplinary approach to management of cases. The fact that you learnt a particular skill or knowledge doesn't mean that another profession can't add anything to make a patient better. This cuts across all professions. A physiotherapist will convince a patient with arthritis that only exercise is what will help you. An orthopedic surgeon will tell the patient that surgery is all that is needed, a pharmacist will assure the patient that the medicine will do magic. The list goes on.... It is until the option has failed that you decide to refer to another professional. The new government has a number of challenges to face within the Health and Social Care sector. The key areas worth highlighting are: 🟢 the pay restoration for junior doctors. 🟢 addressing the ever-increasing waiting times for elective procedural care. 🟢 embracing prevention and lifestyle medicine as a cornerstone in the management of chronic illness. 🟢 expanding the supply of social care. 🟢 investing in hospital infrastructure and expansion. As an interventional cardiologist working within the NHS, I have become increasingly frustrated at seeing patients re-present with further episodes of heart attacks while the management of risk factors remain sub-optimal. In addition, as the waiting lists for interventional procedures grow, these patients remain at an increased risk of suffering an acute event or even death. The concept of prehab is worth reflecting upon. It is used within the physiotherapy setting to prepare patients for their orthopaedic surgery. A similar strategy could be deployed for patients awaiting cardiac procedures. Patients could be directed towards a lifestyle medicine and preventative care programme with a focus on modifying their risk factors while they wait for their procedure. This ‘pre-intervention’ rehab would be a constructive way in managing the waiting lists with the added benefit of increasing the likelihood for long-term procedural success. The MY NEW SELF programme is a lifestyle medicine and preventative care programme offering a holistic approach to cardiovascular health and wellbeing. Such a programme would be ideal for patients to engage with, prior to their procedure as well as providing support to facilitate their post-procedural recovery.
