MyCare Now - Changing One Life at a Time. Hospice is a win too

Sharing another MYCare story shared by one of our dedicated NPs at a rural Pennsylvania nursing facility. Not every story has a rosy ending but referring someone to hospice is oftentimes the best outcome.

Recently I was called to see a young 80 y/o male that had been in and out of the hospital with advanced dementia. He was admitted to the nursing facility for rehab services.  One evening about 5:30, I received a call asking if I could see the patient as he has a fever of 101.4.   I ordered IV’s, stat labs and some other testing all which were negative.  His wife was present during the televisit and we discussed his condition and ordered treatments.
Approximately 10 days later he again started running a fever, was non-responsive to the staff.  I phoned the wife at home and spoke with her about his prognosis and the fact that I did not think he would improve.  I was on the phone with her for over an hour as she cried and told me about his life.  Sometimes (if the patient is comfortable) the family needs us for emotional support and reassurance.  When we consulted hospice, she was ready and at peace with the choices she had made.

This patient likely would have bounced back and forth between the hospital and SNF, further compounding his health issues and contributing further to his decline. Not sure where this would have ended up, but having a telepractitioner in this facility has certainly improved care for one grateful family.
