MyCare Stories - Prolonging life. One Wedding at a time.

Today's MyCare story is shared by Denise, our devoted Tele-NP seeing our patients in rural nursing homes in Pennsylvania and Ohio.

Was called to see a 73 year old female with a history of Congestive Heart Failure who had developed two gastrointestinal bleeding sites. She had been sent to the hospital for surgery to stop these bleeds, however, she went into cardiac arrest during the procedure and continues to have one bleed that is now inoperable. 
She was initially set up to have blood transfusions every two weeks but I was informed after the first transfusion that the transfusion center decided that she would not receive any more.
This patient has a goal of seeing her nephew get married in early fall and so we enrolled her in hospice with the understanding that they would provide the transfusions as ordered, but it has been a constant struggle to get her scheduled as an outpatient. I am monitoring her labs weekly and call hospice often to ensure that she reaches her goal. Her attitude is fantastic, her spirit is upbeat. 
I will continue to push so that she can reach her goal. The staff at the facility has been awesome in helping with this plan.

Taking care of a terminally ill patient is always a delicate balancing act. Having access to daily telemedical care allows her nursing facility to care for her onsite and without having to transfer her needlessly back and forth to the hospital.
