Start a claim

We're here to help you file your claim as quickly and easily as possible, online or by mail. If you have any questions or concerns, our customer service professionals are always available.

Initiate a claim online

To initiate your claim online, we’ll ask you a few questions about your loved one, the circumstances of their passing, and details about yourself. After the claim is submitted, we will contact the beneficiary directly if we need additional details.

Start a claim

To make the process easier, please have the following information ready:

  • Your loved one’s first and last name.
  • Their identifying information, including Social Security Number, Date of Birth, or their New York Life policy number. Using this form will initiate reports on Life and Annuity products, as well as any additional policies they may have owned.
  • Details about their passing, including the date (or best estimate) and cause of death.
  • Any additional documentation you may be able to provide.

Initiate a claim by mail

To initiate your claim by mail, simply download and complete the appropriate form, then mail it to New York Life. After we receive your paperwork, a dedicated New York Life professional will handle the rest. If any additional details are needed, we will contact the beneficiary directly.

I am the beneficiary of a life insurance policy

I am the beneficiary of an annuity policy

Please complete both forms if you are the beneficiary of both a life insurance policy and an annuity policy. If you are not sure about which claim form to use, please contact a Customer Service Professional.

When you call, please have the following information available:

  • Deceased Policy Owner's Name
  • Last Known Address
  • Date of Birth
  • Date of Death
  • Social Security Number (not required)
