The Diabetes Support Program at Orlando Health Physician Associates provides education, skills and activities to help you manage diabetes to help you maintain a positive lifestyle and maximize your quality of life.

Accredited by the Association of Diabetes Care and Education Specialists, the Diabetes Support Program is designed to empower you to manage diabetes for long-term health. Working together with our certified diabetes care and education specialist, you will learn more about how to:

  • Make informed decisions
  • Solve problems
  • Set personal goals
  • Plan ahead
  • Cope with emotions and stress

The program includes:

  • Carbohydrate-counting information
  • Continuous glucose monitoring education, including glucometer instruction
  • Insulin education, including insulin-pump instruction

To Enroll

To enroll, you will need:

  • Referral from a provider, confirming a diagnosis of pre-diabetes or diabetes
  • Copy of your most recent laboratory results
  • Copy of your provider’s most recent office notes

Insurance Coverage

Group and individual consultations are available. Most insurance plans, including Medicare Part B, cover diabetes support services. Insurance plans typically cover up to 10 hours of diabetes education (either individually, in a group or both) during the first year in which you have been referred. Coverage may vary. Please contact your insurance plan directly to confirm your own coverage and plan benefits.
