    APPENDIX D DISHARGE PLAN PATIENTS OUTCOME CRITERIA After the patient is discharge from the hospital, the patient and her family are expected to understand the conditions experienced by the patient and identify measures in helping in the prevention of complications. ASSESSING Assess for signs and symptoms of infection. NURSING ORDER

    Explain to the patient and significant others of the signs and symptoms of infection such as purulent discharge on the site of surgical incision, redness, and swelling.

    PLANNING Promote wellness and wound healing

    Instruct patient and significant others on aseptic technique in cleaning the wound.

    IMPLEMENTING 1. Medication Premature discontinuation of treatment when client begins to feel well may result in return of infection. 2. EXERCISE Avoid heavy lifting or other strenuous exercises.

    Emphasize the use of antiviral/ antibiotics as prescribed.

    Educate the patient to avoid strenuous exercises to prevent the opening or laceration of the surgical incision site.

    3. TREATMENT Explain the take home medication.

    Instruct the patient to take the prescribed medication about when to discontinue medicines.

    Provide health teachings of the advantages and disadvantages of every topic.
