My goal must be based on my learning needs. Your learning goal must be based on your current practice setting and one of

Medication Practice Standard and Directives Practice Guidelines


As a nursing pre graduation student I want to be competent in administering injections like insulin,
heparin and antibiotics by the parenteral routes namely intramuscular and subcutaneous routes as
applicable to the present clinical placement. I will achieve this goal by February 26,2015

Goal #1: Activities and Timeframes


How am I going to achieve my goal? Identify a minimum of three (3) learning activites with
timeframes/timelines
1. February 7: Read and familiarize witth practice guideline Medication 2014.
2. February 10: Observe preceptor administering the injection and watch online videos to
familiarize with the correct technique for injections.
3. February 13: I will be able to give an insulin injection independently and confidently in the
presence of my preceptor
4. February 26: I will be able to administer injections both sc and im to more than one client
independantly

Time Management and organization skills
I want to accomplish the best strategy for organizing care plan for 4 clients so as to complete my
documentation in a timely manner. I want to be able to achieve this by the March 31, 2015

Goal #2: Activities and Timeframes


How am I going to achieve my goal? Identify a minimum of three (3) learning activites with timeframes/timelines.

March 3: Familiarize with practice guidelines on Documentation and Meditech


documentation system in the unit and practice entering data in a fixed allocated time in
order to pick up speed.
March 13: Shadow preceptor step by step so as to pick up the trick of the trade of how to be
quick and efficient in documentation nursing care activity.
March 17: Organize my client care activity in a timely manner so as to document as the
interventions are performed
March 31: I will be able to prioritise care plan for 4 clients and document the same. All
documentations will be completed 15 minutes before the end of the shift. I will be able to
hand over the report to the incoming nurse.

How do my learning goals support my commitment to continuing competency?


How does my learning relate to the competencies of my practice?
Continue to maintain and update your Learning Plan throughout the year

As a student nurse not yet licensed, I intend to take up a part time position as an unregulated
nurse so as to keep in touch with the techniques and skills I have picked up during my pregrad
clinical placement. Continuing competency can only be implemented when a nurse is in
continuous learning process and applies it to practice. This creates quality nursing practice and
earns the respect of the public towards this profession.

Evaluation of changes/outcomes to my practice


What did I learn? What impact has my Learning Plan had on my practice?

By observing my preceptor and other nurses prioritize their nursing care along with
documentation being done as it happens, I realized that I can also implement the same. I used
to finish all my nursing care for all 4 clients and then I would sit to document. This was not a
good practice and I have learned that it is faster to document as the care happens. There was
also the fact that the documentation had numerous sets to be completed for each patients. I
learned that I do not have to go through them all one by one but rather I can choose more than
3 or 4 subsets like vital signs, safety, ADL, mobility and pain. Then I can document them at
one time without struggling for each one individually. This saves time.
The learning plan has met my needs of confidently administering medications and organizing
and prioritizing my nursing care so that I am able to complete my documentation in a timely
manner.
My practice has been observed by my preceptor in day shift comprising of caring for 4 clients
as well as in the night shift caring for 7 to 8 clients.
