    time line given by ESIC doctors.

    Respected Sir,
    This is to inform you that Mr. Tej Singh S/o Mr. Devi Lal R/o G-11, Sec-25; Noida got
    admitted in emergency to Prakash hospital for fifteen days with C/o Sever pain,
    Swelling and bleeding from the left leg. He was an old case of fracture Tibia in left
    leg and diagnosed as case of inter locking nail tibia (Old) with cellulites with
    infected wound in left leg have got operated on 19 th Aug 2013 and discharged on
    21st Aug 2013
    Mr. Tej Singh who is working with our organization since 2011(Cosmic Structures
    Ltd.) is eligible for getting the benefit of ESI and covered under the ESIC act.
    Further Mr. Tej Singh was instructed by ESIC doctor that he would be operated on 2 nd
    Sep 2013 but due to emergency he had to get operated on 19 th Aug 2013. Wherein
    the case was so critical then why did the ESI doctor gave the date of 2 nd Sep 2013
    and instructed to him to get operated on the same day.
    Total expenses incurred by the hospital during the hospitalization is around Rs.
    62000/- which should not be done if an employee is covered under the ESIC and
    facilitated by the ESIC doctors.

    I request you to please take care this case on urgent basis and reimburse the above
    mention amount.
