PROTOCOL DURING SUICIDAL

LEVEL 1

Students who have valid information regarding Suicide Attempt concerning one

1. College Dean; or
2. Department Chairman;
or
3. Professors

of their classmate should immediately inform the College Dean or Department


Chair or Professors whoever is available. The Level 1 Officer-in-Charge (OIC)
shall obtain the contact numbers of the student concerned and his parent from
the Students Information System (SIS).
In specific cases, the Level 1 OIC must coordinate with specific offices such
as University Security Offices (USO) to assist in giving immediate
responses.

LEVEL 2
1. Guidance Office;
and
2. Univ. Health
Services (Clinic)

The Level 1 OIC must report the incident to Guidance


Office and Clinic. The Level 2 OIC will give professional
counseling and intervention to the concerned student.
The same shall also make an official report and
recommendations to address the problem of the student
concerned.

LEVEL 3
Executive
Committee

The official report and recommendations


shall be submitted to the Executive
Committee for their further assistance to
be extended to the student concerned.
