Family Support Grant (FSG)


What?
The FSG is a state program through the MN Department of Human Services that provides cash grants of up to $2,936 per year (as of 1/1/12) to families of children with certified disabilities. These grants are issued to help keep children in the home and access services and supports focused on the family. The FSG has to be used to purchase services and items that are not covered by Medical Assistance or private insurance. These items may include specialized equipment, day care, respite care, education services, computers, and more. The parents will need to submit a copy of their annual taxes during the application process. Once approved, parents are required to complete a plan and budget for the FSG funds. Parents are also required to submit an annual report that includes all receipts for services funded through the FSG.

My two boys have trouble seeing in dimly lit areas so we used the grant to pay for lights to be installed in our closets. Who?
Families with a certified disabled biological or adopted child under the age of 21 living at home are eligible for the FSG. The child must qualify for Medical Assistance and have gone through the State Medical Review Team for approval. Families of children residing in a group home or facility who could return home if given the grant are also eligible. Families must have an annual adjusted gross income of $93,611 or less (as of 1/1/12) to be eligible unless extreme hardship can be shown. Families receiving the Community Alternative Care (CAC), Community Alternatives for Disabled Individuals (CADI), and Traumatic Brain Injury (TBI) waivers are also eligible. Families receiving the Developmental Disability (DD) waiver are not able to receive both the waiver and the FSG, but can still apply. If approved for the FSG, they must discontinue receiving services under the DD waiver.
