    To be accomplished and submitted to the RVP through the HCDMD Chief on a monthly basis. Kindly write in a bulleted format. Summary of Events: Used ICARES in answering clients inquiry on their members profile and contribution history. Issued PHCF1 for members without MDR Called HCDMD/ SMS Command Center for inquiry on doctors accreditation (accreditation number and validity) Informed qualified members on requirements on how to register for Lifetime Member Program Encouraged Non-Member to be a member. Informed them on registration procedure, requirements, benefits in general, eligibility, and contact information of nearest PhilHealth Office Gave out leaflets Accomplished manual tally sheets Problems Encountered / Untoward Incidents/ Actions Taken: Last May 9, 2013 around 11am PhilHealth Section of the Hospital called me through telephone and verified eligibility requirements of a client for a medical case. According to her the member resigned from work recently and currently the client is Individually Paying Member. I told her that since the member is IPP we will be requiring 3/6 and the qualifying quarter since it is already May, we need Jan-March 2013. The members sister went to my desk and told me that she is also working in PhilHealth Central Office and told me that my orientation is wrong because according to her 3/6 is by month . She told me that she have PAR/RF1 for the month of November 2012, January 2013, & February 2013. I explained to her that we are informed that 3/6 for IPP (1 quarter) I also offered her if she want I will call the Benefit administration Section of LHIO-QC to verify but she told me she wants to call Maam Sheng instead. She went away after and went back to the

    PhilHealth Section. I called BAS and verified it again to them, they told me that contribution is lacking of March 2013. I went to PhilHealth Section and they informed me that they will just accept the claims since the PhilHealth Supervisor of the hospital knows them. I texted the relative and informed her that since her sister just shifted recently from Employed-IPP they can asked the PhilHealth Office if they can still pay for the month of March. She replied and told me that she will just ask the employer to pay for month of March since according to her the member is still working during that time. Recommendations: More leaflets General Assembly/Meeting regarding new circulars & upcoming programs
