PETITION
PETITIONER, through counsel, respectfully avers:
1. That Petitioner is a Filipino, of legal age, single, and a resident
of 195 Osmena St., Cabuyao, Laguna, Philippines, where he
may be served with summons and other processes of this
Court;
2. That the respondents are the public official-in-charge of the
custody, issuances and release of Certificates of Live Birth in
Lopez, Quezon whose official address is THE OFFICE OF THE
CIVIL REGISTRAR, Lopez, Quezon, and the NATIONAL
STATISTICS AUTHORITY located at Sta. Mesa, Manila, where
it may be served with notices and other processes of this
Honorable Court;
3. That the petitioner, ANTHONY ESPONERA RONQUILLO was

born on March 30, 1991 at PHO-Laguna Provincial Hospital
located at Sta. Cruz, Laguna;
A faithful reproduction of his Certificate of Live Birth with
registry no. 91-1258 issued by the Local Civil Registrar of Sta.
Cruz, Laguna, is hereto attached as Annex A and is an
integral part of this petition.
4. That on August 9, 1997, his mother BELINDA E. RONQUILLO
got married to his step-father, DANILO J. ESCOBIDO in
Cabuyao Laguna;
5. That the couple decided to register again herein petitioner
ANTHONY ESPONERA RONQUILLO with the Civil Registrars
Office of Lopez, Quezon, this time using the name ANTHONY
RONQUILLO ESCOBIDO
A faithful reproduction of the Certificate of Live Birth of
Anthony Ronquillo Escobido with registry no. 98-2377 is
hereto attached as Annex B and is an integral part of this
petition.
6. That sometime in 2013, petitioner sought the correction of his
first birth certificate that indicated ANTONIO ESPONERA
RONQUILLO by changing his first name from ANTONIO to
ANTHONY. That was approved on January 14, 2014
pursuant to the decision rendered by MCR Laudemer F. San
Juan and affirmed by CRG under OCRG No. 14-1125507 as
per sub-marking A-1 of Annex A;
7. That Mr. Ronquillo was able to get a passport from the DFAManila with Passport No. EB7604009 issued on March 11,
2013 under the name ANTHONY RONQUILLO ESCOBIDO, a
faithful reproduction of it is hereto attached as Annex C;
8. That on May 26, 2016, petitioner tried to apply for a passport
using his original birth certificate (Annex A, supra) under the
name ANTHONY ESPONERA RONQUILLO via the e-passport
facility, a printout of said application is hereto attached as
Annex D;
9. That on the same day, he was referred to the DFA-Office of

Consular Affairs Aseana for further evaluation because of the
discrepancy of his name;
A faithful reproduction of the DFA Referral slip is hereto
attached as Annex E and is an integral part of this petition.

That after the evaluation by the DFA-Office of Consular
Affairs Aseana, he was required to submit the following: 1)
Court Decision re: Cancellation of Second Birth Certificate, 2)
Old documents with correct date and place of birth and, 3) an
Affidavit of Explanation so that his application can be
processed; and
11.
That as a consequence, his passport which was to expire
on March 10, 2018 yet, was blocked and cancelled.
PRAYER
WHEREFORE, it is most respectfully prayed of this Honorable
Court, that after due notice, publication and hearing, an ORDER be
issued directing the Office of the Civil Registrar of Lopez, Quezon to
CANCEL the Certificate of Live Birth issued in the name of
ANTHONY RONQUILLO ESCOBIDO as he is the same person
named ANTHONY ESPONERA RONQUILLO.
Such other reliefs just equitable under the premises are
likewise prayed for.
