    applicable
    Consignees name, complete street address including zip code and
    phone number including area code
    Issuing carriers agent name and city, if applicable
    Agents IATA code
    Agents AA Cargo account number
    Accounting information (i.e. third party billing information)
    Airport of departure and requested routing
    Three letter destination airport code for first transfer point
    Name of first carrier
    Three letter destination airport code for second transfer point
    Two letter code for second carrier
    Three letter destination airport code for third transfer point
    Two letter code for third carrier
    Airport of destination
    Requested flight and date
    Shippers internal reference number (i.e. house air waybill)
    Three letter ISO currency code (from country of departure)
    Type of payment (code entered by AA Cargo)
