    Dear

    This pertains to the resolution (Submission of Investigation Report with


    Recommendation) rendered by the Investigation Committee which provides that:

    Pursuant to Section 18 of the Revised Rules on Administrative


    Cases in the Civil Service, the Members of the Investigation
    Committee, after conducting a preliminary investigation or ex parte
    examination of records and documents submitted by the
    complainant and the person complained of, as well as documents
    readily available from other government offices, unanimously find
    prima facie case against the respondent for the charge/s imputed
    against her, and it is respectfully recommended that a formal charge
    or notice of charge be issued and given to respondent and to
    proceed in accordance with the existing rules.

    After a careful scrutiny of the records, the undersigned hereby issues the formal
    charge for the above- mentioned administrative offenses against respondent and
    orders the Investigation Committee to conduct a Formal Investigation pursuant to
    Rule 8 Section 30 of the 2011 Revised Rules on Administrative Cases in the Civil
    Service.

    The instant formal charge, being a part of due process is being issued to afford both
    parties an opportunity to be heard in the formal investigation/hearing to be
    conducted by the Investigation Committee, and should not be interpreted as neither
    an imposition of penalty nor diminution/curtailment of rights, benefits, and
    privileges.

    Accordingly, considering that the merits of the case cannot be decided judiciously
    without conducting a formal investigation let a Formal Investigation be conducted
    by the Investigation Committee pursuant to Rule 8 Section 30 of the 2011 Revised
    Rules on Administrative Cases in the Civil Service.
