PETITION
PETITIONER, JUAN DELA CRUZ, to this Honorable Court,

most respectfully files this Petition for Renewal of his
Commission as a Notary Public for Cebu City, Carcar City and
the Municipality of San Fernando, Cebu, for a term
commencing on 1 January, 2018 and ending on 31 December,
2019. In support of this petition, he most respectfully states,
THAT:
1. He was previously granted a Notarial Commission by

Order dated September 18, 2016 signed by Executive
Judge Gilbert P. Moises. Attached is a copy of the Order
marked as Annex “A”.
2. He possesses the eligibility for commissioning as Notary

Public shown by his personal circumstances listed below:

5. He has not been convicted, much less, charged with any

offense involving moral turpitude;

commencing on September 29, 2016 and ending
on December 31, 2017, marked as Annex “K” and

k. Clearance issued by the Executive Judge,

marked as Annex “L”.
8. Other than this Petition, Petitioner has not applied for a

Notarial Commission nor was he issued a Commission in
any Province or City in the Philippines outside the
jurisdiction of this Honorable Court for the current year;
9. Petitioner possesses all the qualifications and none of the

disqualifications and thus, Petitioner desires to be
commissioned as a Notary Public for the Cities of Cebu

and Carcar and the Municipality of San Fernando within
the jurisdiction of this Honorable Court.
PRAYER
WHISEFORE, premises considered, Petitioner most

respectfully prays of this Honorable Court to GRANT herein
Petition and that Petitioner be APPOINTED AS NOTARY PUBLIC
for Cebu City, Carcar City and the Municipality of San
Fernando, Cebu.
