    JOINT AFFIDAVIT OF TWO DISINTERESTED PERSONS

    We, ________________, of legal age, Filipino, and both with residence and postal address at
    _______________ Pateros, Metro Manila, after having been duly sworn to in accordance with the
    law, do hereby depose and state that:

    1. We personally know ___________________, of legal age, Filipino and a resident


    of Pateros. She is our neighbor and have known her since childhood;

    2. We recently learned that she has no birth record with the National Statistics
    Office (NSO). Attached is a copy of Certification of Non-Availability of Birth
    Record.

    3. We hereby certify that she was born on ________________ in Pateros, Metro


    Manila;

    4. She has been using said birth date and place in all her documents and
    transactions;

    5. We are not related by affinity of consanguinity to _____________; and

    6. We are executing this affidavit to attest to the truth of the foregoing facts and for
    whatever legal intents and purposes it may deem serve.
