    AFFIDAVIT OF SOLO PARENT

    I, Kobe Bryant, of legal age, Filipino, single and resident of LM


    Subdivision, Sta. Filomena, Guagua, Pampanga, after having been duly
    sworn to in accordance with law, hereby depose and say, that:

    1. I had a relationship with one Kobe Bryant. We never got married and are
    now separated for more than twelve (12) years.

    2. During the relationship, we begot one child named Kobe Bryant who was
    born on February 14, 2007;

    3. Since Kobe Bryant and I separated, I was left as the parental custodian
    of our children, and to solely shoulder my and our children’s financial
    needs and support;

    4. I am executing this affidavit to attest to the truth of the foregoing and attest
    to the truth and veracity of all the foregoing and for all legal intents and
    purposes it may serve.

    IN WITNESS WHEREOF, I have hereunto set my hand this 31st day of


    Ocotber 2018 in Guagua, Pampanga Philippines.
