    I, _______________, of legal age, Filipino, and single, after having


    been duly sworn to in accordance with law, hereby depose and say, that:

    1. That I am the private complainant in _____________ against the


    ___________________ for the crime of VIOLATION OF BATAS
    PAMBANSA BLG. 22 (4 counts) now pending before
    _____________iled against the aforesaid accused for the reason that I
    came to realize that the same arose from misunderstanding and
