January 12-13, 2016.
I am aware of the usual risks and dangers inherent in the said contest thus, accidents can occur
with or without any fault on either the part of the student, or the teachers where the contest is taking
place. By allowing my child to participate in this activity, I accept the risk or any untoward incident, and
