    MOTION NUNC PRO TUNC

    COMES NOW PETITIONER VILLAMOR U. LICAYO,


    through counsel, unto this Honorable Court, alleges the
    following:

    1. Petitioner filed a petition for correction of name from


    Villamor Obinia Licayu to Villamor Ubinia Licayo on
    ______ to this Honorable Court.

    (Attached hereto is the Petition filed marked as Annex A)

    2. The Honorable Court rendered a decision in favor of


    Petitioner on August 8, 2011 with dispositive portion
    states:

    “WHEREFORE, in view of all the foregoing, the petition is


    hereby GRANTED. Corollary, thereto, the Local Civil
    Registrar Piat, Cagayan is hereby ordered to make the

    following corrections in the record of birth of petitioner,
    to wit:
    1. Name : From Villamor Obinia Licayu to Villamor
    Obinia Licayo
    x x x
    3. Mother’s Family Name: From Obinia to Ubinia
    x x x

    SO ORDERED.”

    (Attached hereto is the Decision of the Honorable Court


    dated August 8, 2011 marked as Annex B)

    3. There being no motion or appeal filed by the parties


    within the reglementary period, the decision has become
    final and executory.

    (Attached hereto is the Certificate of Finality marked as


    Annex C)

    4. Perusal to the said Decision and to the Certificate of


    Live Birth, petitioner observed that his middle name was
    not changed to Ubinia. And it was only his mother’s
    family name was changed from Obinia to Ubinia.

    (Attached hereto is the Certificate of Live Birth marked as


    Annex D)

    PRAYER

    PREMISES CONSIDERED, it is moved that the name of


    petitioner be corrected from Villamor Obinia Licayo to Villamor
    Ubinia Licayu from the Decision of this Honorable Court.
